With Week 12 of the 2019 head-to-head fantasy baseball season upon us, it's time to consider some waiver wire pickups that can make a difference in lineups this coming week.

Here are five fantasy baseball waiver wire pickups that could be the difference between winning and losing your Week 12 matchup.

5. Jason Heyward, OF, Chicago Cubs

Jason Heyward has always been a streaky hitter and he's on one of those tears right now. Heyward has nine hits (including a home run) and five runs scored in his last eight games. He's also available in 80 percent of Yahoo Fantasy leagues, so if you're in need of some outfield help, Heyward's recent hot streak makes him well worth an add for Week 12.

4. Martin Perez, SP, Minnesota Twins

Martin Perez has quietly been fantasy worthy all season. When he's not getting wins, he's racking up strikeouts and sometimes he gets you both. With a 7-2 record and a 3.97 ERA, the left-hander also has 67 strikeouts in 70 1/3 innings pitched. Perez only gets one start this week, facing Kansas City on Friday, June 21, but he's very playable for that game.

3. Josh Reddick, OF, Houston Astros

Josh Reddick has been a consistent source of production this season. The veteran outfielder has hit .300 this year and has eight home runs and 55 combined runs and RBI while batting toward the top of the first-place Astros' lineup. Reddick is certainly worth an add for any outfielder-needy fantasy owner.

2. Ryan Pressly, RP, Houston Astros

Another Astro worth adding is relief pitcher Ryan Pressly. The right-hander has quietly been extremely impressive for Houston this season, pitching in 30 games while allowing just four earned runs in 31 1/3 innings pitched. He's also struck out 34 batters and has a 0.67 WHIP. Currently available in 51 percent of Yahoo! Fantasy Baseball leagues, Pressly can help you in lots of pitching categories out of the RP slot each and every week.

1. Yandy Diaz, 1B/3B, Tampa Bay Rays

Yandy Diaz has been heating up since returning from the injured list at the beginning of June. While his season totals alone should be enough to add him in fantasy (.282 AVG, 11 HR, 30 RBI), what he's done lately has been even more impressive. Diaz already has 20 hits and five multi-hit games in June and is hitting over .350 during that span. Diaz, who is available in 62 percent of Yahoo! Fantasy Baseball leagues, should be a top priority must-add for owners who can only make moves to start the week.
